Who can receive hospice care?
Hospice care is available to anyone with a life-limiting illness who chooses comfort-focused care. It supports patients of all ages and medical conditions. Essentials Hospice evaluates each patient’s needs and creates a personalized care plan.
When should someone consider hospice care?
Hospice is appropriate when curative treatments are no longer effective or desired, and comfort and quality of life are the main focus. Families often consider hospice when symptom management and support are needed. Essentials Hospice helps guide families through the timing and decision-making process.
How does hospice support families?
Hospice provides counseling, education, and bereavement support for families before, during, and after a patient’s passing. Our team helps families navigate difficult decisions and emotional challenges. Essentials Hospice continues support for up to a year after the patient’s death.
Eligibility and Referral procedure for Hospice Care
- Upon meeting the following criteria, eligibility requirements will be determined for a patient.
- Based on the clinical judgement of the Primary Physician and the Hospice Medical Director, it has been certified that the patient has a limited life expectancy.
- The patient’s Primary Physician authorizes hospice care
- The focus is now on physical, emotional, and spiritual comfort rather than curative measures
What is the admission process?
Admitting a patient to the hospice program is a team process for the attending physician, discharge planner or social worker and Essentials Hospice. A nurse from Essentials Hospice will visit the patient and family at home , Skilled Nursing Facilities, Residential or Assisted Living Centers or in the hospital, where information about the program will be presented. If the patient qualifies and elects the program, the hospice team makes all necessary arrangements for admission.

a. Fees for services provided to patients who are covered by insurance plans are billed directly by the agency to appropriate insurance company.
b. Private Payments- All patients are billed directly for services that are not reimbursed by insurance.
